# Bleeding risk mechanisms and reversal strategies for factor Xa inhibitors

## Abstract

Bleeding with factor Xa inhibitors occurs predominantly as intracranial hemorrhage (64-69%) or gastrointestinal bleeding (23-52%) in elderly patients (mean age 76-78 years) with substantial comorbidities, including kidney insufficiency in 48% and atrial fibrillation as the primary anticoagulation indication in 81%. Andexanet alfa, administered as a bolus followed by 2-hour infusion, reduces anti-factor Xa activity by 92-94% for apixaban and rivaroxaban and achieves excellent or good clinical hemostasis in 67-82% of patients. In the randomized ANNEXA-I trial, andexanet demonstrated superior hemostatic efficacy compared to usual care (primarily 4F-PCC) for intracerebral hemorrhage (67.0% vs 53.1%), but was associated with higher rates of thrombotic events (10.3% vs 5.6%), particularly ischemic stroke (6.5% vs 1.5%). Thrombotic events consistently occurred before anticoagulation resumption, with no events after oral anticoagulation restart, indicating that thrombotic risk results from interruption of anticoagulation rather than rebound effects. Reduction in anti-FXa activity was modestly predictive of hemostatic efficacy specifically in patients with intracranial hemorrhage and correlated with lower mortality in younger patients, suggesting that the benefit of aggressive reversal is context-dependent and modified by patient age and bleeding location.

## Methods

We analyzed 10 sources from an initial pool of 200, using 8 screening criteria. Each paper was reviewed for 8 key aspects that mattered most to the research question.

## Bleeding Risk Mechanisms

Patient-specific risk factors for bleeding with FXa inhibitors included advanced age, significant comorbidities like kidney insufficiency (48%), diabetes (28%), prior stroke (23%), and heart failure (20%). In patients with gastrointestinal bleeding, 86% had baseline hemoglobin levels ≤10 g/dL, suggesting pre-existing anemia as a potential risk factor.

The short half-lives of FXa inhibitors may influence the need for aggressive reversal strategies. In patients with intracranial hemorrhage, increased bleeding risk relates to hematoma expansion. The proposed pathophysiological mechanisms involve activation of coagulation pathways and the intrinsic thrombotic risk of the patients.

## Reversal Strategies

### Agent Selection and Dosing

Reversal strategies varied, primarily utilizing andexanet alfa and 4-factor prothrombin complex concentrate (4F-PCC). Andexanet alfa was administered as a bolus followed by an infusion, with dosing dependent on specific FXa inhibitors and timing since the last dose.

### Timing of Reversal

Reversal was initiated within 18 hours of FXa inhibitor administration in the ANNEXA-4 studies. In the ANNEXA-I trial focusing on intracerebral hemorrhage, patients were enrolled within 15 hours of taking a FXa inhibitor before bleeding onset.

## Laboratory Reversal Efficacy

Andexanet alfa demonstrated substantial reductions in anti-factor Xa activity across all studied FXa inhibitors. The initial bolus produced median reductions of 89% for rivaroxaban and 93% for apixaban, although reductions returned to baseline levels four hours after the end of the infusion. In comparative studies, the median reduction in anti-factor Xa activity was significantly higher with andexanet.

## Clinical Hemostatic Efficacy

Hemostatic efficacy was assessed using standardized scales, with excellent or good outcomes achieved in 79-82% of evaluable patients across the ANNEXA-4 studies. A significant difference in hemostatic efficacy was observed between andexanet (67.0%) and usual care (53.1%) in the ANNEXA-I trial, emphasizing the context-dependent nature of reversal strategies.

## Safety Outcomes

Thrombotic events represented the primary safety concern following reversal of FXa inhibitors, with rates varying significantly across studies. The mortality rates also varied, reflecting the severity of intracranial hemorrhage in the patient population. Notably, thrombotic events predominantly occurred prior to anticoagulation resumption, underscoring the timing challenges in resuming anticoagulation safely.

## Synthesis

The evidence indicates a complex relationship between laboratory reversal of FXa inhibitors, clinical hemostasis, and thrombotic risk that necessitates careful interpretation across clinical contexts. A nuanced approach to the balance between bleeding control and thrombotic risk is paramount.
